Job summary

TeamCare Medical in New York is seeking a Clinical Pharmacist Case Manager to work with prescribers and the IDT, enhancing care for PACE participants with complex regimens.

You will assess patients for optimization, resolve drug-related problems, and collaborate with the Pharmacy Benefit Manager to ensure Part D compliance and cost-effective medication use. Strong communication and autonomy are essential. Occasional travel within sites may be required.

Qualifications

• Graduate of an ACPE-accredited School of Pharmacy with a BS in Pharmacy or PharmD.
• PGY1 Pharmacy Practice Residency with a focus in ambulatory care or geriatrics preferred.
• NY State Pharmacy license required.
• Board Certification in Geria tric or Ambulatory Pharmacy preferred.

Responsibilities

• Reviews and evaluates drug information to identify medication therapy problems and recommends appropriate solutions to the IDT.
• Conduct comprehensive medication reviews for participants to optimize care outcomes.
• Monitor prescribing patterns across multiple sites and provide education to staff as needed.
• Develop and implement policies related to medication utilization with emphasis on quality, safety, and cost effectiveness.
• Provide in-service education to internal clinical staff on medications and develop patient-centered education for PACE participants.
• Maintain a standardized stock medication list and tracking system at each PACE site.
• Collaborate with Pharmacy Benefit Manager to ensure Part D compliance and cost-effective medication utilization.
