Job summary

The Clinical Pharmacist Case Manager at C2Q Health Solutions collaborates with prescribers and the IDT to optimize medications for PACE participants with multiple chronic conditions, assess treatment options, and address drug-related problems. They work with the Pharmacy Benefit Manager to ensure regulatory compliance and cost-effective use of medications.

Qualifications

• Graduate of an ACPE accredited School of Pharmacy with a BS in Pharmacy or PharmD required.
• PGY1 Pharmacy Practice Residency with a focus in ambulatory care or geriatrics preferred.
• NY State Pharmacy license required.
• Board Certification in Geriatric or Ambulatory Pharmacy preferred.
• Minimal one year managed care pharmacy experience; one year ambulatory care; one year long term care/consulting.

Responsibilities

• Reviews and evaluates drug information to identify potential medication therapy problems for IDT and suggests solutions.
• Conduct comprehensive medication reviews for participants to optimize care outcomes.
• Monitor prescribing patterns across sites and educate staff; audit records and report to leadership.
• Implement policies and SOPs for medication utilization with focus on safety and cost effectiveness.
• Review literature and develop new pharmacy strategies and pilot projects.
• Serve as a clinical pharmacy resource for technicians and staff.
• Develop patient-centered, literacy-level appropriate education for PACE participants.
• Provide in-service education to internal clinical staff on medications.
• Maintain standardized stock medication list and tracking at each PACE site.
