Job summary

Community LIFE is seeking a Clinic Nurse (RN) to join our team in Homestead, PA. The position is full-time with hours 7:30am-4pm on weekdays and on-call rotation. The RN will assess needs, coordinate clinic services for frail elderly patients, and may conduct home visits.

Requirements include graduation from an accredited nursing school, PA RN license, and a PA driver’s license. At least one year of relevant experience is preferred, with strong communication and organizational skills.

Qualifications

• Graduate of an accredited nursing program.
• Minimum of 1 year experience with frail/elderly populations; long-term care/home health preferred.
• Licensed RN in PA and valid PA driver’s license.

Responsibilities

• Assess patient needs and coordinate delivery of clinic services.
• Provide care to frail or elderly population; may perform home visits.
• Communicate effectively with patients, families, and interdisciplinary team.
