Job summary

Qualifications

• RN or equivalent nursing licensure required.
• CPR certification required.
• Experience in outpatient case management preferred.
• Proficient with electronic medical records and email communication.

Responsibilities

• Provide follow-up care to identified patients based on complexity and social determinants of health.
• Coordinate care with post-discharge concerns or barriers.
• Provide transitional care to risk-stratified patients after discharge.
• Ensure patients understand their medication list and education.
• Determine frequency of telephone encounters based on patient needs.
• Identify barriers to care and engage appropriate resources.
