Job summary

UCLA Health is seeking a Senior Care Physician to join our Senior Care Model clinic in Encino. You will deliver patient-centered primary care for adults 65+, leveraging the Age-Friendly Health System’s 5Ms framework and collaborating with an interdisciplinary team to support geriatric care.

The role focuses on longer visits, a smaller panel, and ongoing professional development in chronic care management and population health strategies.

Qualifications

• Board certified/eligible in Internal Medicine or Family Medicine, required.
• Board certified/eligible in Geriatric Medicine, preferred.
• California licensed or licensed-eligible, required.
• Passion for delivering patient-centered geriatric care and improving health outcomes for older adults.
• Ideal candidates will be comfortable with telehealth appointments, as needed.

Responsibilities

• Provide comprehensive primary care for a smaller patient panel to enhance patient access and personalized care.
• Enjoy sharing quality visit times with your patients with longer visits.
• Work in a collaborative, interdisciplinary team including Clinical Advisor RNs, Comprehensive Care Coordinators, and Licensed Clinical Social Workers to support patient care.
• Dedicate 32 hours per week to direct patient care, with additional time allocated for administration and chronic care management.
• Develop professionally through specialized education focused on chronic care management, value-based care, and population health strategies.
• Collaborate with the Senior Care Model Medical Director to drive quality improvement initiatives and evaluate new care delivery models.
