The position features a base salary in the range of $270,000–$300,000 with potential quality-based incentives and total compensation up to $450,000

Qualifications

• Specializes in geriatric primary care for older adults.
• Comfort with telehealth appointments as needed.
• Collaborates with Clinical Advisor RNs and social workers.

Responsibilities

• Provide comprehensive primary care for adults 65+, focusing on high-value, patient-centered care.
• Deliver 32 hours per week of direct patient care with additional admin time.
• Engage in care-model initiatives and quality-improvement projects.
