Job summary

Asian Health Services seeks a PACE Intake Coordinator to help older adults and caregivers access comprehensive, coordinated care under the PACE model. You will determine eligibility, gather medical and social information, and set intake appointments by phone, in the home, or at the PACE Day Center.

You will manage referrals, conduct pre-enrollment screenings, support LOC assessments, and prepare clinical summaries for the IDT.

Qualifications

• 2+ years of clinical nursing experience (geriatrics, primary care, home health, SNF, hospital case management).
• Active California RN license in good standing.
• Graduate of an accredited School of Nursing.
• Current BLS certification (or ability to obtain).
• Valid driver’s license and ability to travel for home visits.

Responsibilities

• Receive, triage, and track referrals; prioritize by acuity and capacity.
• Conduct or coordinate initial clinical screening to confirm eligibility.
• Review medical records to support eligibility and IDT review.
• Coordinate pre-enrollment assessments (nursing, social work, therapies).
• Perform home visits to assess care needs and safety barriers.
• Complete LOC assessments and prepare clinical summaries for IDT.
• Coordinate handoff from intake to ongoing care teams upon enrollment.
