Job summary

All Seniors Foundation in Los Angeles, CA, seeks a Memory Care Coordinator to design and coordinate our dementia care program for seniors with Alzheimer’s and dementia, partnering with families, clinical teams, and community partners to deliver coordinated care across in-home and community settings.

You will lead caregiver support groups, coordinate memory care resources, and train staff on dementia communication and safety.

Qualifications

  • Must hold a bachelor’s degree in gerontology, social work, nursing, or related field.
  • At least 2 years working with dementia patients and families.
  • CDP certification or willingness to obtain within 6 months.
  • Strong knowledge of dementia stages, behavior, and care strategies.
  • Group facilitation skills.

Responsibilities

  • Coordinate care plans for seniors with dementia and their family caregivers.
  • Conduct cognitive screenings and refer for diagnostic workups when appropriate.
  • Lead caregiver support groups and dementia-friendly community programming.

Role Overview

As Memory Care Coordinator, you will design and coordinate our memory care program for seniors with Alzheimer’s, dementia, and mild cognitive impairment. You will work with families, clinical teams, and community partners to deliver coordinated dementia care across in-home and community settings.

Key Responsibilities
  • Coordinate care plans for seniors with dementia and their family caregivers
