Job summary

NEHCR represents a leadership opportunity in Flushing, NY for a Care Delivery Manager - Registered Nurse. You will oversee daily clinical operations, coordinate services, and support culturally competent, person-centered geriatric care in a PACE model.

Requires MLTC experience, strong community health/home care background, and an active NY RN license with supervisory experience. Bilingual skills are a plus, with preference for BSN and additional case management training.

Qualifications

• Active, unrestricted New York State RN license and registration.
• Managed Long-Term Care (MLTC) experience required.
• Supervisory experience required.
• Minimum of 2+ years of administrative/leadership experience in CHHA/LHCSA/MLTC settings preferred.
• Case Management Certification recommended within 2 years of employment.

Responsibilities

• Oversee development and execution of individualized care plans.
• Lead and participate in IDT meetings ensuring compliance and timely documentation.
• Provide clinical leadership within IDT discussions.
• Review Clinical Delivery reports to drive care coordination.
