Job summary

Elder Services of Cape Cod and the Islands is seeking a Geriatric Support Services Coordinator to coordinate care within the Senior Care Options team. The role focuses on helping SCO and One Care members access services that improve quality of life, with strong coordination across departments.

Qualifications

• Ability to collaborate, take initiative, provide/receive feedback.
• Ability to work independently and professionally.
• Excellent computer skills required, including MS Office 365.

Responsibilities

• Coordinate support services as part of the SCO Primary Care Team.
• Assist eligible SCO and One Care members to obtain services which enhance quality of life.
