Qualifications

• Clinic, post-acute or long-term care experience in last 5 years.
• Health, dental & vision insurance provided.

Responsibilities

• Develop and manage comprehensive care plans with patients, families, physicians, and teams.

• Provide health education to patients and families to manage chronic conditions.
