Job summary

Carease Health is seeking a Care Companion/Social Worker in Wisconsin to support residents in skilled nursing facilities. You will provide emotional support, brief therapeutic interventions, and mental health assessments as part of a Collaborative Care team.

The role involves home and facility visits, coordination with psychiatric professionals, and regular supervision. This is a part-time position with approximately 10 hours per week, Monday through Friday, within 8 am–5 pm hours.

Qualifications

• Geriatric mental health understanding and crisis management in long-term care settings
• Strong communication, documentation, and relationship-building skills
• Education: Associate Degree
• Experience: 1 year

Care Companion/Social Worker at Carease Health - WI is seeking a passionate and experienced Care Companion/Social Worker to join our Collaborative Care team. In this role, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. As a key member of our team, you'll work with psychiatric professionals to ensure comprehensive care, promoting the well-being and quality of life for our patients.

Key Responsibilities:

• Visit designated facilities in accordance with the schedule set with your supervisor

• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs

• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns

• Document visit notes and maintain detailed records of interactions and interventions after each visit

• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications:

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field

• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

• Strong communication, documentation, and relationship-building skills

• Education: Associate Degree

• Experience: 1 year

• Part-time opportunities with approximately 10 hours per week during Monday-Friday, within regular 8-5 hours

• Flat salary: $17,000 - $19,000 ANNUALLY
