Job summary

Qualifications

• Must read, write, and follow oral instructions.
• High school education or equivalent preferred.
• Certification process must be completed within state deadlines.
• Must be able to provide care as outlined on the Interdisciplinary Care Plan.

Responsibilities

• Sign in on Unit Assignment Sheet.
• Review changes on Group Assignment Sheets.
• Assist residents with bathing, toileting, grooming, and daily care.
• Assist residents to meals and monitor hydration.
• Document observations on Nursing Flow Record and other records.
• Maintain infection control practices and a clean, safe environment.
