Job summary

Harbor Health operates the Elder Service Plan in Brockton and seeks a Center Aide/CNA for the Adult Day Health Center in Brockton. The role is 30 hours weekly, Monday through Friday 8:30 AM – 3:00 PM, delivering personal care and support to geriatric participants.

Requirements include a HS diploma or GED, CNA or HHA certification, current BLS, and 1–2 years of geriatric experience preferred; dementia training preferred.

Qualifications

• High School Diploma or GED required.
• CNA or Home Health Aide certification required.
• Current BLS certification required.
• 1–2 years of geriatric experience preferred.
• Dementia training preferred.

Responsibilities

• Provides personal care and assists with activities and social needs of geriatric participants.
• Assists with mobility and meal setup/feeding as needed.
• Provides physical and emotional care for participants.
• Escorts participants to medical appointments.
• Maintains safe clinic environment and supports daily operations.
• Collaborates with nursing staff for assessments and issue identification.
• Attends multidisciplinary team morning and Care Plan meetings.
