This role includes routine rounds, proactive chronic disease management, and close collaboration with interdisciplinary teams and facility partners.

The ideal candidate has current NP/PA licensure in the state, experience in geriatrics or internal medicine, and strong patient-centered communication and teamwork skills.

Qualifications

• Current NP or PA licensure in the state of practice.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Previous professional patient care experience.
• Exceptional patient-centered service skills and ability to build trusting relationships with residents, families, and facility staff.
