Job summary

Eventus WholeHealth seeks an Advanced Practice Clinician (NP or PA) to deliver comprehensive primary care in skilled nursing and senior living communities. You will conduct routine rounds, manage chronic diseases, and collaborate with interdisciplinary teams and facility partners to promote proactive, value-based care.

Qualifications

• Current NP or PA licensure in the state of practice.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills and communication.
• Ability to collaborate across interdisciplinary teams.

Responsibilities
