Eventus WholeHealth is seeking an Advanced Practice Clinician (NP/PA) to provide comprehensive primary care in skilled nursing and senior living communities. You will conduct routine rounds, manage chronic diseases proactively, and coordinate care with interdisciplinary teams and facility partners.

The role emphasizes preventive care, improved outcomes, and a better patient and family experience, with collaboration across care teams to deliver integrated, person-centered plans.

Qualifications

• Current NP or PA licensure in the state of practice.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams

Responsibilities
