Job summary

Liberty Medical is seeking an experienced and compassionate Adult Nurse Practitioner (ANP) to join our outpatient medical practice in Holmdel, NJ. This position focuses on geriatric medicine and patient-centered care in an outpatient setting.

The NP will evaluate, diagnose, and manage older adults with chronic disease management, preventive care, and overall wellness, collaborating with physicians and the healthcare team to support health, independence, and quality of life.

Qualifications

• Current APRN license in New Jersey.
• Board certification as a Nurse Practitioner.
• Geriatric/Adult-Gerontology/Family NP experience preferred.
• Experience caring for older adults with multiple chronic conditions preferred.
• DEA registration and prescribing privileges.
• Ability to work independently while collaborating with physicians.

Responsibilities

• Perform comprehensive medical evaluations and physical examinations.
• Diagnose and treat acute and chronic conditions.
• Manage and monitor medications with appropriate adjustments.
• Provide preventive care, screenings, and health education.
• Assess functional status, fall risk, and cognitive concerns.
• Coordinate care with physicians, specialists, and families.
• Order and interpret laboratory tests and other evaluations.
• Maintain accurate electronic medical records.
• Participate in quality-improvement initiatives and team collaboration.
