Job Summary
  • The Services Coordinator provides administrative and clinical supervision for a team of supervisors and performs all administrative duties required to adhere to the guidelines of the program.
  • The Services Coordinator monitors all programs for compliance to HRMC P&P and funder expectations.
  • He/She is responsible for representing her programs, as well as, the center at community meetings in a positive and professional manner.
  • He/She is responsible for monitoring all contracts for productivity compliance.
  • Ensures coverage for program vacancies and is able to perform front line duties as needed.
JOB DUTIES/RESPONSIBILITIES

1. New Program Development
  • Research and identify gaps in service delivery system as exhibited by reporting service needs in supervision.
  • Successfully implement new programming within HRMC as evidenced by 3 months start up to full implementation.
2. Compliance
  • Ensure all programs meet standards of licensing agent and funder as evidenced by successful audits (less than 5 finding on each audit including but not limited to PAR, ASQ, TDMH, Title 1, DOE, United Way, Scorecard, CARF)
  • Ensure all programs meet standards of licensing agent and funder as evidenced by no repeat finding in audits.
3. Quality Programming
  • Continual improvement in quality programming as evidenced by the identification and correction of problem areas with no reoccurring issues
  • Identify best practice standards for program areas as evidenced by publication in program manuals
  • Implement best practice standards for program areas as evidenced by staff training.
  • Implement best practice standards for program areas as evidenced by integration into service delivery (i.e. curriculum development and implementation)
  • Has a working knowledge of Performance Based Contracting as evidenced by assertive utilization review as documented in the internal DCS database
  • Has a working knowledge of Performance Based Contracting as evidenced by an average length of stay equal to or less than the current fiscal year baselines
  • Has a working knowledge of Performance Based Contracting as evidenced by adoptions and permanent exits that are equal to or greater than the performance outlined in the BTA's
  • Is fully engaged in client outcomes as evidenced by participation and advocacy in CFTMs to move clients quickly, though appropriately, toward permanency as indicated by completion of treatment goals and completion of core programs requirements.
Is Educated on and implements Department of Education Standards as evidenced by:
  • clients getting correct testing,
  • course completion (credits)
  • improvement in Math and Reading scored,
  • parent involvement
  • HISET attainment
  • Graduation from high school
Implements and monitors life skill, vocational and parenting programs as evidenced by:
  • Improvements in Ansell Casey scores
  • Completion of career assessments
  • Completion of vocational assessmentsCompletion of application for housing the day of the client's 18th birthday
  • Weekly life skill education groups
  • Completes all required clinical documentation in accordance with Center Policy and Procedure and funding source guidelines.
  • Completes comprehensive treatment plan with first 30 days of admit with all appropriate signatures.
  • Completes all Quarterly Treatment Plans every 3 months on each client with all appropriate signatures provided.
  • Completes Discharge Plan 45 days prior to discharging from the Program when applicable.
  • Completes opening paperwork (releases, CPC if applicable, consents, sex abuse packet, program rules, etc.) within first 30 days of treatment.
  • Completes Contract and Foster Home Checklist the day of admission.
  • Completes Progress notes within P&P guidelines.
  • Completes monthly Utilization Reports by the 5th day of the following month.
  • Returns required documentation to appropriate administrative personnel as required by TFACTS (includes Foster Home Checklists, Contracts, admit/discharge confirmations, census reports, job requests).
  • Communicates case updates to supervisor for Utilization review meetings
  • Complete and fax Serious Incident Reports within 24 hours to all necessary individuals at DCS.
  • Provides and helps foster parent complete all paperwork (med logs, progress notes, allowance contracts).
  • Completes 15 hours of training each year.
  • Is an effective Team Member
  • Responds to all emails and voicemails within 24 hours.
  • Attends and is on time for mandatory tx team meeting weekly.
  • Participates in staff development activities as requested by supervisor.
  • Participates in supervision with immediate supervisor.
  • Takes on additional responsibilities when asked (extra clients, covers co-workers caseloads, committees, etc.).
4. Public Relations
  • Represents the Center in the community in a positive and professional manner as evidenced by good working relationships (satisfaction survey, referrals, and complaints).
  • Liaison and communicates with DCS staff including placement and Regional Administrators to facilitate good working relationships as evidenced by
  • No complaints to Director by DCS
  • An obvious relationship with leaders at DCS that are located in the Regions
  • Presence and active participation in provider meeting (i.e. cross functional, quarterly meetings, QSR)
5. Personnel
  • Ensure positions are filled in a timely manner as evidenced by position filled within 2 months
  • Staff issues are identified and addressed promptly as evidenced by the utilization of progressive disciplinary procedures.
  • Staff are supervised on a regular basis (preferable weekly) and have a good working knowledge of job duties as evidenced by clear, professional documentation of staff strengths and areas of concern with a detailed action plan to address areas of concern
  • Foster professional growth and development of staff through the identification of leadership potential.
6. Productivity
  • Maintain productivity to ensure all revenue targets are met (including earning cost based grants).
  • Control expenses (target of under 97%) in relationship to revenues earned.
COMPENSATION:
  • Starting salary for this position is approximately $75,037 /yr based on relevant experience and education.
