Job summary

Nuvance Health Med Practice CT is seeking an LPN to support clinicians in patient exams at multiple CT locations. The role includes preparing patients, gathering histories, assisting with exams, and administering medications under supervision. Must maintain compliance and patient safety.

The position offers full-time hours, with a sign-on bonus for external candidates. Travel between practices is required, and benefits are provided in line with the organization’s policies.

Qualifications

• High School diploma or equivalent.
• Active LPN License required by state; BLS certification within 30 days.
• Previous medical office experience preferred.

Responsibilities

• Prepares materials, supplies, equipment, and the patient to assist clinician with the medical exam.
• Obtains patient history and medical information.
• Provides direct patient care as directed by a Registered Nurse or clinician.
• Collects specimens and performs point of care testing.
• Administers medications and vaccinations as directed; educates patient on care and follow-up.

Summary:

To assist the clinician in providing medical care to the patients of the Medical Practice.

Responsibilities:

1. Prepares materials, supplies, equipment, and the patient to assist clinician with the patient's medical exam. 2. Obtains the patient's history and medical information. 3. Provides direct patient care as directed by a Registered Nurse or clinician. 4. Collects and processes specimens as directed by the clinician and performs point of care testing. 5. Administers medications, and vaccinations as directed by the clinician. Instructs the patient and family on how to perform clinician recommended preventive care, takes prescribed medication, prepares for tests, monitors treatment progress and notes symptom changes. 6. Contacts the patient to provide requested follow-up, advises them of test results or relays clinicians instructions. 7. Fulfills and adheres to all compliance responsibilities related to the position. 8. Maintains and Models the Organizations values. 9. Demonstrates regular, dependable, and predictable attendance. 10. Performs other duties as required.

Education Skills Experience:

High School diploma or equivalent. Desired: Previous medical office experience preferred.

Other Information:

Ability to travel between practices as needed. Required: Active LPN License based on state; BLS certification required within 30 days.
