Qualifications

• Physician licensed to practice medicine in the current state.
• Licensed by the DEA and the State of Illinois to prescribe narcotics, if applicable.
• Board certified or board eligible in the appropriate specialty.
• Eligible to participate in Medicare, Medicaid and other federal programs.
• Maintains medical staff membership and privileges and professional liability coverage.
