Job summary

Phoenix House Orange County, Inc. in Sylmar, CA is seeking a Wrap Facilitator (Full-time) to lead the Child and Family Team and manage a 10–12 client caseload. Position requires a BA/BS in Human Services or related field and relevant wraparound experience.

Qualifications

• Bachelor of Arts or Science in Human Services, Social Work, or Psychology.
• Three years of experience working with high-risk children/youth in wraparound or related services.
• Must have a valid California Driver’s License, proof of automobile insurance, and a safe driving record.
• Must obtain TB test clearance and update annually.

Responsibilities

• Facilitate the Child and Family Team meetings and complete required documentation.
• Assess and document client needs and strengths per contractual standards.
• Participate in treatment team meetings to update treatment plans and supports.
• Provide crisis intervention as needed, including evenings, nights, and weekends.
