CommuniCare Rehab in Kokomo, IN is hiring a Speech Language Pathologist (SLP) for a full-time role at Healthcare Center. This position is in-house with opportunities to treat residents across multiple locations and contribute to a collaborative rehab team.

Responsibilities include evaluating medical charts, developing treatment plans for communication, swallowing, and cognitive needs; leading therapy sessions; instructing residents and families; participating in interdisciplinary meetings; and

Qualifications

• Bachelor’s and/or Master’s degree in Speech-Language Pathology from an accredited program.
• Holding a valid or pending State Speech-Language Pathology license.

Responsibilities

• Evaluate, review, and screen patient charts and develop treatment plans per physician orders.
• Plan and conduct rehabilitative programs to improve communication, swallowing, and cognitive skills.
• Instruct residents and families on practice strategies and safe swallowing techniques.
• Lead interdisciplinary meetings and relay clinical findings to the rehab team.
• Treat residents at multiple locations to meet diverse care needs.

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Plan, direct, or conduct rehabilitative treatment programs to improve communication, swallowing, and/or cognitive problems, as well as administer, score, and interpret specialized speech tests.
• Instruct residents, families, and caregivers to monitor speech and provide ways to practice new skills, diet recommendations, and modifications for safe swallowing techniques.
• Attend/Lead interdisciplinary meetings as appropriate to relay clinical findings and make recommendations to other members of the rehab team, as well as participating in family meetings/education.

Qualifications

• Must have a Bachelors and/or a Master’s Degree in Speech-Language Pathology from an Accredited Program.
