Responsibilities

• Create individualized treatment plans tailored to each resident's functional goals.
• Provide therapy services for post-acute, neurological, and medically complex patients.
• Collaborate with nursing, rehab staff, physicians, and families to support continuity of care.
• Monitor resident progress and maintain accurate clinical documentation.
