Meridian Health Services in Madison County, IN is seeking a full-time day Skill Builder I for our children's services program. The role provides skills and development training to assist patients and/or families in achieving developmental milestones while supporting the clinical team with case management and care coordination.

The ideal candidate will have a high school diploma with 2 years of paid behavioral healthcare experience (associate or bachelor's preferred), a valid driver's license,

Formación

• High school diploma or equivalent required with 2 years paid behavioral healthcare experience.
• Associate or Bachelor's degree preferred.
• Valid driver's license with driving record meeting Meridian policy, reliable transportation.
• Commitment to providing individual attention to help clients achieve well-being.

Responsabilidades

• Assists the Therapist in developing, implementing and revising individual treatment plans; monitors progress toward treatment goals.
• Provides case management and coordinates care with clinical team.
• Works with patients and families to develop skills for daily living and coping with behavioral health challenges.
• Maintains clinical records, reports and forms; supports coordination with other agencies.
