Job summary

AdvisaCare is seeking a Registered Nurse Case Manager (RNCM) to coordinate compassionate hospice care in patients’ homes and care communities. You’ll assess needs, develop care plans, and provide nursing care with a patient-centered approach.

Join a team focused on meaningful work, ongoing training, and strong support. The role requires a current Michigan RN license, hospice experience preferred, and comfort with daily field visits and on-call responsibilities.

Qualifications

  • Current Michigan license in good standing.
  • Minimum 1 year hospice experience preferred.
  • Empathetic approach to patients and caregivers.
  • Participate in field visits daily, office meetings as indicated.
  • Ability to adapt to patient’s status and needs on a full time on call basis.

Responsibilities

  • Assess and support patients and families by completing thorough initial and ongoing assessments to understand their hospice care needs.
  • Provide compassionate, professional nursing care while using the nursing process to ensure each patient receives individualized and high-quality care.
  • Develop and coordinate individualized care plans by creating and initiating the patient’s plan of care and regularly reassessing needs.
  • Provide medications and treatments as prescribed by the physician and outlined in the plan of care.
  • Educate and guide patients and families on nursing care and other healthcare needs.
  • Provide clear health education to patients and families based on their plan of care.
  • Teach patients and families practical self-care techniques for home care.
  • Collaborate with the interdisciplinary team to deliver well-rounded hospice care.
  • Contribute to quality improvement meetings and activities.

Essential Job Functions
  1. Assess and support patients and families by completing thorough initial and ongoing assessments to understand their hospice care needs. This includes reviewing their current health status and medical history.
  2. Provide compassionate, professional nursing care while using the nursing process to ensure each patient receives individualized and high-quality care.
  3. Develop and coordinate individualized care plans by:
  • Creating and initiating the patient’s plan of care.
  • Regularly checking in and reassessing the needs of the patient and their family/caregiver.
  • Working with the care team to update the plan of care as the patient’s needs change.
  • Provide medications and treatments as prescribed by the physician and outlined in the patient’s plan of care.
  • Support and guide patients and families/caregivers by providing education, encouragement, and guidance related to nursing care and other healthcare needs.
  • Provide clear and helpful health education to patients and families based on their individual needs and plan of care.
  • Teach patients and families/caregivers practical self-care techniques and ways to feel more confident managing care at home.
  • Take opportunities to educate and support patients and families while working closely with the interdisciplinary care team to provide well-rounded, compassionate hospice care.
  • Contribute to quality improvement efforts by actively participating in quality assessment and performance improvement meetings and activities.
    • Current Michigan License in good standing
