Responsibilities

• Conduct comprehensive nutrition assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized nutrition care plans.
• Evaluate nutritional status and provide recommendations to support resident health and recovery.
• Collaborate with physicians, nursing staff, speech therapists, and dietary personnel to ensure optimal resident outcomes.
• Monitor residents for significant weight changes, nutritional risks, and dietary concerns.
• Participate in care plan meetings and interdisciplinary team discussions.
• Ensure compliance with state, federal, and facility regulations regarding nutrition services.
• Provide nutrition education and counseling to residents, families, and staff as needed.
• Support the Dietary Services department in maintaining high-quality nutrition programs.
