Job summary

CareMed Health Solutions, a facility-based provider serving residents in long-term care facilities in Ohio, is seeking a Nurse Practitioner to deliver primary and preventive care.

You will manage residents with acute changes, participate in advanced care planning, collaborate with interdisciplinary teams, and prescribe medications in accordance with OH law. A state NP license and board certification are required, with a $5,000 sign-on bonus for full-time eligibility.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.
• Psych experience a plus

Responsibilities

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.

• Manage Residents with acute change in condition.

• Advanced care planning and chronic care management.

• Prescribe appropriate medications, including controlled substances in accordance with OH Law.

• Order, review and/or follow-up with treatments and diagnostic testing.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

• and/or the care plan.

• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS
