Job summary

CareMed is seeking a full-time Nurse Practitioner to provide primary and preventative care for residents in skilled nursing facilities in the Lima/Paulding area. You will manage acute changes, perform advanced care planning, and coordinate chronic care management with an interdisciplinary team.

Requirements include an OH NP license, board certification, DEA licensure, and experience with long-term care. EMR proficiency and collaboration with primary care physicians are essential; geriatric

Qualifications

• Current, unrestricted NP license in OH
• Board Certification in NP
• DEA licensure
• Clinical background in adult, family or geriatric specialties
• Experience in long-term care settings
• Ability to work with an EMR
• Ability to manage patients with advanced illness and multiple chronic conditions
• Ability to work within an interdisciplinary care team
• Psych experience a plus

Responsibilities

NP ROLE AND RESPONSIBILITIES

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS
