CareMed Health Solutions is seeking a qualified Nurse Practitioner to provide primary and preventive care to residents in long‑term care facilities in Kentucky. You will collaborate with an interdisciplinary team and manage chronic and acute conditions within an EMR.

Ideal candidates hold an unrestricted KY NP license, board certification, and DEA licensure, with geriatric or family practice experience in long-term care.

Qualifications

• Current, unrestricted NP license in KY.
• Board Certification as an NP.
• DEA licensure per KY law.
• Clinical background in adult, family or geriatric care.

Responsibilities

• Provide primary and preventative care for long-term care residents.
• Manage residents with acute changes in condition.
• Advance care planning and chronic care management.
• Prescribe medications, including controlled substances per KY law.
• Order, review and follow-up with treatments and testing.
• Collaborate with nursing home staff and the primary care physician.
• Coordinate care within an interdisciplinary team.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

Np Qualifications And Education Requirements

• Current, unrestricted NP license in the state of KY
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus
