Job summary

Prime Healthcare Illinois Medical Center- St. Mary of Nazareth -Chicago, IL is seeking a dedicated Nurse Practitioner to join our outpatient Primary Care team in a full-time role. You will provide patient-centered care in a busy clinic alongside physicians and other APPs.

Qualifications

• Licensed and certified as an NP in Illinois.
• Board-certified in Family Medicine or Adult-Gerontology.
• Prior experience in primary care preferred but not required.

Responsibilities

• See patients for preventive care, chronic disease management, and acute visits
• Collaborate with physicians, APPs, and clinical staff
• Manage own patient panel with continuity of care
• 40 hour/week schedule with minimal call
