Job summary
Moraga Post Acute is hiring a Licensed Vocational Nurse (LVN) for the Full-Time PM shift in Moraga, CA. The LVN will provide compassionate nursing care to residents following individualized care plans, administer medications, monitor conditions, and collaborate with a multidisciplinary team.

Qualifications
- Current and valid California LVN license.
- Experience in skilled nursing or long-term care preferred.
- Strong clinical and communication skills.
- Ability to work with an interdisciplinary team.
- Detail-oriented with accurate documentation.
Responsibilities
- Provide high-quality nursing care per individualized care plans.
- Administer medications and treatments as ordered.
- Monitor residents and report changes to the healthcare team.
- Document nursing assessments promptly and accurately.
- Collaborate with CNAs, RNs, physicians, and therapists.
- Assist with care plan development and implementation.
- Provide wound care and other treatments as directed.
- Monitor vital signs and clinical indicators.
- Maintain complete nursing documentation.
- Communicate with residents, families, and healthcare providers.
- Promote safety, dignity, privacy, independence.
- Follow facility policies and healthcare regulations.
- Respond to emergencies and changes in resident condition.
- Support nursing assistants and other team members.
- Perform other nursing-related duties as assigned.
