Position Summary: Twin Cities Physicians is seeking a PhysicianAssistant to join our care team in Golden Valley! With Twin Cities Physicians,you will be providing the best care for individuals within all types of seniorliving communities. We work with many communities across the Twin Cities Metroarea, bringing primary care right to our patients on a weekly basis!

Responsibilities:

• Drive to SeniorLiving Communities to provide on-site primary care to residents
  
• Facilitate thepatient's plan of care: chronic care management, advanced treatment, andend-of-life planning
  
• Coordinate withthe interdisciplinary team, reconcile medications, and order labs
  
• Communicateclearly and regularly with Physician, office support staff, communitystaff, and patients' families
  

Qualifications:

• Currentregistration as a Physician Assistant / PA by the MN Board of MedicalPractice
  
• Strongportfolio in Geriatrics, Primary Care, or Internal Medicine
  
• Passion forserving the Older Adult Population
  
• Stronginterpersonal communication skills
  
• Valid driver’slicense and current auto insurance
