Job summary

Northwell Health in New York is seeking a licensed Physical Therapy Assistant to support patient care under the supervision of a physical therapist. You will document progress, participate in care planning, and maintain accurate medical records for each patient as part of a collaborative clinical team.

Responsibilities include delivering direct patient care per prescribed plans, completing documentation, and contributing to quality improvement activities.

Qualifications

• High School Diploma or equivalent required.
• Current License to practice as a Physical Therapy Assistant in New York State required.
• 1-3 years of relevant experience, required.

• May attend family meetings/conferences pertaining to the welfare and management of assigned patients.
