Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following individualized plans of care, and collaborate with facility staff to ensure high-quality outcomes.

In this role you will supervise Physical Therapist Assistants, re-evaluate progress, adjust goals, and complete timely documentation of evaluations, re-evaluations, discharges, and daily notes.

• Re-evaluating patient progress, revising plans of care and resetting the goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
