Job summary

Enhance Rehabilitation seeks a Physical Therapist to join our team in a SNF setting. You will evaluate and treat patients following individualized plans of care, while collaborating with facility staff to ensure excellent patient outcomes.

You will also supervise Physical Therapist Assistants and handle re-evaluations, as well as documenting evaluations, re-evaluations, discharges and daily progress notes. A commitment to high-quality patient care is essential.

• Re-evaluating patient progress, revising plans of care and resetting goals
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
