Wisconsin Illinois Senior Housing, Inc. (WISH), a non-profit organization, is seeking a licensed Physical Therapist to join our dedicated team. In this full-time role, you'll provide rehabilitation services across our Geneva Lake Manor, East Troy Manor, and Holton Manor facilities, with your primary base at Geneva Lake Manor. Collaborate with an interdisciplinary team to develop individualized treatment plans and enhance our residents' quality of life.

Physical Therapists in SNFs play a crucial role in enhancing residents' mobility, independence, and overall quality of life.
Key responsibilities include:
Evaluating Residents: Assessing functional abilities and limitations to develop personalized treatment plans
Implementing Therapy Programs: Designing and executing interventions aimed at improving strength, balance, and mobility
Collaborative Care: Working closely with an interdisciplinary team, including nurses and occupational therapists, to ensure comprehensive care
Documentation: Maintaining accurate records of treatment plans, progress notes, and outcomes
Education: Instructing residents and their families on exercises and strategies to support ongoing rehabilitation
Hours: Flexible:
Monday through Friday
8:00am - 4:30pm

Qualifications:
Graduate of an accredited education course in physical therapy.
Licensed in the state of Wisconsin
