Job summary

CommuniCare Rehab is seeking a Full Time Physical Therapist (PT) for Kenwood Healthcare Center in Cincinnati, OH. Our therapists are in-house team members delivering direct resident care and developing individualized therapy plans.

You will evaluate patients, create and implement treatment plans, supervise PTAs, and participate in interdisciplinary meetings to ensure comprehensive care. Multiple center assignments may be available to meet diverse patient needs.

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Responsibilities

• Evaluate, review, and screen patient’s/resident’s medical charts and develop a treatment plan per physician order.
• Implement treatment plan through direct treatment and educate the plan of care to all team members; supervise PTAs as needed.
• Lead interdisciplinary meetings to relay clinical findings and make rehab recommendations; train staff and families when appropriate.
