Job summary

Warsaw Rehabilitation And Healthcare Center in Warsaw, NC is hiring a Part-Time Physical Therapist to join our growing team. You will conduct comprehensive PT evaluations, develop treatment plans, document progress, and collaborate with the interdisciplinary team to meet patient goals.

The role requires a PTA/PT license in good standing, experience with MD orders, and the ability to work within a team to provide patient-centered care in a long-term care setting.

Qualifications

• Bachelor's or higher in Physical Therapy or equivalent as recognized by APTA.
• Current license/registration to practice physical therapy in state.

Responsibilities

• Provide comprehensive PT evaluations based on MD orders.
• Document findings on standardized evaluation forms.
• Develop comprehensive treatment plans with goals, frequency and duration.
• Obtain MD orders for evaluations, treatments, re-certifications and discharges.
• Deliver treatments using modalities and adjust as indicated.
• Document daily participation and weekly progress.
• Update treatment plans and discharge summaries as needed.
• Document caregiver or discharge in-services as required.
• Complete facility documentation and MDS-related info.
• Attend rehab and facility meetings as directed.
• Participate in scheduling and patient screens.
• Assist in billing and bill patients accurately for PT services.
• Cover other therapists’ caseload during absences.
• Supervise junior staff or students when needed; co-sign notes.
• Maintain professional relationships with staff, patients and families.
• Provide in-service education on patient care and PT services.
• Comply with all facility policies, confidentiality, and HR rules.
• Maintain punctuality, professionalism and a positive work atmosphere.
• Perform any other duties assigned by supervisors.

Education

Doctorate in Physical Therapy (APTA-recognized)
