Job summary

Warsaw Rehabilitation And Healthcare Center in Warsaw, NC is seeking a full-time Physical Therapist to provide comprehensive PT evaluations and develop individualized treatment plans in collaboration with the care team.

The position requires graduation from an accredited PT program and an active North Carolina license, with a focus on delivering high-quality, patient-centered care and meeting physician orders and discharge criteria.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD’s orders.
• Document findings on the standardized evaluation form, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify treatment as indicated while adhering to precautions.
• Document patient’s daily participation and progress on weekly progress note accurately and timely.
