Job Specific Accountabilities

Accurately assesses patient

• Performs and documents physical therapy evaluations on each patient referred for therapy. Performs and documents reassessment of patient's function including changes in patient's condition and treatment response within specified time frames.
• Assesses patient's functional mobility and need for assistive devices.
• Assesses educational needs based on age specific differences.
• Assesses anticipated discharge setting.

Properly performs physical therapy treatment.

• Performs and documents an in-depth history to include chief complaint, health status and information pertinent to each patient for therapy.
• Prescribes a program of corrective exercise and treatment for assigned patients, including manual techniques.
• Performs such treatments as exercise, gait training, functional training, massage, manual techniques, joint mobilization, whirlpool, hot and cold packs, ultrasound, paraffin, and traction.
• Explains treatment to be provided, expected reaction or sensation, and duration of treatment sessions.
• Secures necessary equipment and supplies for treatment.
• Documents effectiveness of care and documents updates of the plan of care based on the challenging needs of the patient.
• Provides care within the established time frame.
• Coordinates patient care with the other health team members

Pay:

$41.43 - $51.79 per hour

Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for CHLB/CMC, including but not limited to shift differential and other special pay practices, etc. The posted compensation for the position is a reasonable estimate that extends from the lowest to the highest pay that CHLB/CMC in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
