• Develop and implement personalized physical therapy treatment plans for patients to achieve specific goals and outcomes.
• Conduct thorough evaluations and assessments to identify physical therapy needs and develop treatment plans.
• Deliver high-quality physical therapy services, including but not limited to exercises, modalities, and manual therapy.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions and coordination.
• Document patient progress and treatment plans accurately and in a timely manner.
• Participate in quality improvement initiatives to enhance patient outcomes and facility operations.
