Flagship Therapy is seeking a dedicated Full Time Physical Therapist for Malley Transitional Care in Northglenn, Colorado. The role involves working in an in-house rehab program serving geriatric patients across short-term, long-term, and post-acute care pathways.

Qualifications

• Licensed Physical Therapist required.

Responsibilities

• Screen and evaluate patients with physical functioning disorders.
• Deliver rehabilitative, skilled, and medically necessary treatments.
• Assist with standing, walking, lifting, and positioning as needed.
• Administer rehabilitation procedures including manual techniques, exercises, modalities, and wound care.
• Provide counseling to patients and families about physical disorders.
• Generate discharge summaries complying with regulatory requirements.
