Concept Rehab is hiring a Full-Time Physical Therapist (PT) for Hart, MI to deliver patient-centered care in a supportive, clinician-led environment. Responsibilities include evaluating patients, developing individualized plans, and supervising PT Assistants while contributing to discharge planning and care coordination with the interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Michigan Physical Therapist license.
• Strong clinical and communication skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled physical therapy services.
• Help patients improve strength, balance, mobility, and functional independence.
• Monitor patient progress and modify treatment plans as needed; collaborate with nursing, physicians, families, and the interdisciplinary team.
• Supervise and collaborate with Physical Therapist Assistants.
• Participate in care planning and discharge planning.
• Educate patients, families, and facility staff on appropriate therapeutic programs.
• Complete accurate and timely clinical documentation.

What You'll Do
