Full Time Physical Therapist - $5k Sign-On Bonus

Skilled Nursing Facility: A leading provider of rehabilitation services for the elderly with many locations throughout the country. Our dedicated professionals are committed to enhancing our patients' quality of life with passion and integrity.

The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides Physical Therapy services including treatment planning & implementation, and discharge planning. Tests patients’ physical abilities, analyzing rehabilitation goals; consults with the rehab team to coordinate the most effective physical therapy program.

Organizational Relationships: Supervises: Responsible for the care delivered by PTAs, Rehab Technician.

1. Evaluate patients/residents within 24 hrs of physician referral.
2. Develop effective treatment plans and obtain approval for services from referring physician, and treat patients/residents according to physician’s treatment plan.
3. Communicate regularly with supervisor, other members of the rehabilitation team, and nursing staff on the patient’s treatment plan, progress, and prognosis.
4. Record evaluations, daily treatment notes, weekly progress notes, 14-day progress report to the physician, and discharge summaries per policy and procedure.
5. Instruct patient/resident’s families or nursing staff in maintenance program.
6. Secure necessary durable medical equipment for patients/residents to facilitate independence in mobility: i.e. orthoses, walkers, canes, elevated toilet seats, and grab bars.
7. Submit logs and all billing information on a timely basis.
8. Report any problem areas/equipment within each facility to the appropriate supervisor immediately.
9. Comply with the State Licensing Board of Kentucky, Title 22, CMS, and Kentucky Labor Board Guidelines.
10. May perform other duties as assigned.

Licensure/Certification: Bachelor's degree (or foreign equivalent) in Physical Therapy and a current license or recently completed your exams.
