Part-Time Physical Therapist Contract Position

LifeStream is currently seeking a Physical Therapist consultant to provide evaluation and services to the individuals we support.

Position: 16 hours per week.

Job Duties:

1. Assessment
2. Program development
3. Staff training
4. Progress review
