Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team. As a Physical Therapist, you will provide high-quality care for patients in a Skilled Nursing Facility (SNF) setting, evaluating and treating patients according to individualized care plans.

Responsibilities include:

• Providing excellent patient care
• Delivering outstanding customer service to patients and their families
• Collaborating with facility staff to ensure optimal patient care
• Evaluating new patients, developing care plans, and setting measurable, functional goals
• Treating patients, addressing concerns, and educating them on their injuries, progress, and outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising care plans, and adjusting goals
• Completing timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes
