The Physical Therapist will screen, evaluate, re-evaluate, discharge plans, and revise plans of care under physician supervision, with a focus on quality outcomes and patient engagement.

Qualifications

• Bachelor's, Master's or Doctorate degree in physical therapy from an accredited program.
• Active state license required.
• Certification and licensure requirements as established by state and national board associations.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.
• Ability to supervise Physical Therapy Assistants
• Screening requirements may vary by location.

Responsibilities
