Job summary

Functional Pathways in partnership with the Arizona State Veterans' Home in Yuma, AZ, is seeking a Full-time Physical Therapist to join a dedicated therapy team. You will screen, evaluate, and revise care plans under physician supervision, supporting residents’ rehabilitation and well-being.

The role emphasizes compassionate care, collaboration with a multidisciplinary team, and adherence to FP policies and procedures.

Qualifications

• Bachelor’s, Master’s or Doctorate in Physical Therapy from an accredited program.
• Active state license required.

Responsibilities

• Screen and evaluate residents’ physical therapy needs under physician supervision.
• Re-evaluate and discharge plans of care as appropriate.
• Revise plans of care to optimize rehabilitation outcomes.
• Supervise Physical Therapy Assistants as part of patient care workflow.
