Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatments.
• Stand/walk throughout the facility; occasional lifting, transferring, and repositioning of patients/equipment.
• Administer procedures as part of rehabilitation plan: manual therapy, ambulation, exercises, modalities, devices, wound care.
• Provide counseling to patients, families, caregivers, and other providers.
• Generate discharge summaries ensuring regulatory and clinical compliance.
